Our skilled professionals are highly trained to help each patient safely achieve their maximum level of independence.
In-home skilled services may include:
Nurses are skilled at providing post-operative care, safety evaluations, disease education and management, pain management, wound care, IV therapy, catheter care, and medication management. They act as a liaison between doctors, patients, and their caregivers to ensure that patients receive the necessary care in their home environment.
Physical Therapists evaluate and treat a variety of conditions, such as pain, injury, neurological impairments, or muscle and bone disorders that limit safe mobility. They specialize in joint replacement therapy, walking and balance training, muscle strengthening, and help with proper use of medical assisted devices. They develop an individualized care plan that incorporates exercises to build strength, coordination, balance, and endurance.
Occupational Therapists help patients regain their independence in daily activities, such as bathing, grooming, dressing, housekeeping, and meal preparation. They achieve this by providing treatment to improve functional mobility, including the use of adaptive equipment and home modifications.
Speech Therapists work with patients who experience communication and/or swallowing disorders that accompany diseases such as stroke, MS, and traumatic brain injuries. They also assist with language and comprehension, judgment and memory skills, and enhance written spoken and non-verbal communication.
Social Workers offer assistance to families and caregivers by enhancing coping skills while promoting the social and emotional health of patients. Social Workers have access to a network of community resources to assist in maintaining the patient’s health and safety.
Home Health Aides are critical members of our team who provide personal care assistance, such as bathing, dressing, transferring, toileting, and skin care. They work under the supervision of nurses and/or therapists to provide support and care for patients.
