Advanced Home Health & Hospice’s origins as a company were founded on the belief that a facility exclusively designed and dedicated to providing short-term rehabilitative services would raise the standard of excellence in the post-acute care continuum. In 2004, Advanced Home Health & Hospice constructed the nation’s first free-standing transitional rehab facility in South Ogden, Utah. This new model for providing short-term nursing and rehabilitation revolutionized the way that care is provided to patients and has become the benchmark of excellence for the entire industry.
